It usually happens to me when there's a number of people on or if I have high ping to a server. It can be corrected by activating the chat console and so entering and exiting chat quickly fixes it. I think it has to do with iff you are firing when you are fragged or something; in the script log you can see "bug" and something else ( I will check again) at the time ...
